Worker Injury During Preparation of a Piping System for Pressure Testing at the Hanford Site

On April 25, 2018, the U.S. Department of Energy Office of Enterprise Assessment’s Office of Enforcement issued Preliminary Notice of Violation (WEA-2018-01) to Bechtel National Inc., for violations of DOE’s 10 C.F.R. Part 851Worker Safety and Health Program requirements associated with an event in which a worker was injured when a 12-inch pipe joint separated and released a pressurized stream of water during preparation of a piping system for pressure testing at the Waste Treatment and Immobilization Plant at the Hanford Site. A Final Notice of Violation was subsequently issued to Bechtel National Inc., LLC, on September 12, 2018.
