Associated with a worker injury that occurred during preparation of a piping system for pressure testing at the Hanford Site.
On September 12, 2018, the U.S. Department of Energy Office of Enterprise Assessment’s Office of Enforcement issued a Final Notice of Violation to Bechtel National Inc. associated with an event in which a worker was injured when a 12-inch pipe joint separated and released a pressurized stream of water during preparation of a piping system for pressure testing at the Waste Treatment and Immobilization Plant at the Hanford Site.
