An accident at the Fermi National Accelerator Laboratory (Fermilab) was investigated in which three painters received burns as a result of a flammable liquid fire/explosion while preparing the floor of a storage room for painting. The Accident Investigation Board (the Board) used various analytical techniques, including accident analysis, barrier analysis, and event and causal factor analysis.
