Operating Experience Summaries

The Office of Environment, Health, Safety and Security (EHSS) publishes Operating Experience Summaries (OES) to inform and support DOE facilities and organizations with beneficial DOE or external operating experience-based information.  The OES is one of many products and tools provided by EHSS to support line organizations with tailoring the local implementation of their Operating Experience (OpEx) programs, as defined in DOE O 210.2A, DOE Corporate Operating Experience Program

The Emergency Management Operating Experience Summary provides summaries of findings from two reports published by the Office of Enterprise Assessments (2022 and 2024) and presents enterprise-level insights from the 2025 Annual Report on the Status of the Department’s Emergency Management System. The OES shares tools and information that enable field sites to make tangible updates and bolster the Department’s readiness in effectively responding to evolving operational risks.
The Occurrence Reporting and Processing System (ORPS) is a foundational Department of Energy (DOE) tool that supports leadership awareness, organizational learning, and continuous performance improvement across the DOE complex.
The Computerized Accident/Incident Reporting System (CAIRS) is a database used to collect and analyze Department of Energy (DOE) and DOE contractor reports of work-related injuries, illnesses, and fatalities.
This Operating Experience Summary (OES) provides a synopsis of the Accident Investigation Board (AIB) report developed, when, at Fermi National Accelerator Laboratory, an ironworker fell approximately 23 feet from a concrete wall form, sustaining serious injuries, including head trauma.
