The non-fatal, electrical arc blast accident at the Kansas City Plant on May 24, 1998, occurred due to a lack of effective integration and failure to responsibility implement the high voltage work control process. Deviation from AlliedSignal FM&T/KC’s established electrical safety procedures contributed to this accident.
