A fatal accident at a construction site at the Brookhaven National Laboratory (BNL) was investigated in which a construction laborer was accidentally run over by a heavy construction wheel loader when it backed during a grading operation. In conducting its investigation, the Accident Investigation Board (the Board) used various analytical techniques, including accident analysis, barrier analysis, and event and causal factors analysis.
