On September 21, 2012, at the request of the Bonneville Power Administration (BPA) Chief Safety Officer, a Level I Accident Investigation was convened to investigate an accident in which a BPA lineman was fatally injured as the result of a fall from a tower on the Dworshak-Taft #1 500kV line.
The purpose of the investigation was to determine the cause of the accident and to develop recommendations for corrective actions to prevent recurrence.
