On September 3, 1999, the U.S. Department of Energy issued a Preliminary Notice of Violation (EA-1999-08) to the University of California for violations of 10 C.F.R. Parts 830 and 835 associated with a series of deficiencies in work controls and in radiological monitoring that led to a worker's unplanned, uncontrolled radiological exposure and
intake of radiological material at the  Los Alamos National Laboratory's Chemistry and Metallurgy Research Facility.
