The accident under investigation occurred on April 3, 1995, at approximately 10:46 a.m. As a result of the accident, a Wackenhut Services, Incorporated – Savannah River Site (WSI–SRS) Special Response Team (SRT) member received fatal injuries from a 27-foot fall from the top of the Savannah River Site (SRS) Advanced Tactical Training Academy Security Rappel Tower.
