Worker Toluene Overexposure Event at the Depleted Uranium Hexafluoride Conversion Facility near Paducah, Kentucky

On July 30, 2025, the U.S. Department of Energy (DOE) Office of Enterprise Assessments’ Office of Enforcement issued Preliminary Notices of Violation to Mid-America Conversion Services, LLC, (WEA-2025-01) and subcontractor Omni Services, Inc. (WEA-2025-02) for violations of DOE’s 10 C.F.R. Part 851 Worker Safety and Health Program requirements related to an incident of worker overexposure to toluene at the Depleted Uranium Hexafluoride Conversion Facility. A Final Notice of Violation was subsequently issued to Mid-America Conversion Services, LLC, on December 3, 2025.
