Related to the release of radioactive material from a glovebox in the Plutonium Facility Building 4 (PF-4) at Los Alamos National Laboratory.

On July 19, 2022, the U.S. Department of Energy (DOE) Office of Enterprise Assessments’ Office of Enforcement notified Triad National Security, LLC, of its intent to investigate potential noncompliances with DOE nuclear safety program requirements (10 C.F.R. Parts 830 and 835) involving Triad National Security, LLC. These potential noncompliances are related to a release of radioactive material from a glovebox in the Plutonium Facility Building 4 (PF-4) at Los Alamos National Laboratory on January 7, 2022, which resulted in radiological contamination of personnel and had the potential for significant unplanned intake of radioactive material.
