Issued to Fluor Daniel Hanford, Inc., related to Potential Quality Assurance Rule Violations at the Hanford Site
This letter refers to the Department of Energy's (DOE) investigation of potential violations of the Quality Assurance Rule (QA Rule) nuclear safety requirements identified in the subject reports. Fluor Daniel Hanford, Incorporated, as Management and Integrating contractor for the DOE's Hanford site, has responsibility for assuring sub-tier contractors comply with its Quality Assurance Plan. On April 27 and 28, 1997, the Office of Enforcement and Investigation personnel met with members of your staff and representatives from Lockheed Martin Hanford Company (LMHC). Our purpose was to understand the circumstances leading to the subject noncompliances and progress in carrying out corrective actions. We are including our Investigation Summary Report with this letter.
